Oxandrolone, sold under the brand names Oxandrin and Anavar, among others, is an androgen and anabolic steroid (AAS) based medication that is used for weight gain in various situations, to compensate for protein catabolism caused by long-term corticosteroid therapy, to support recovery from severe burns, to treat bone pain associated with osteoporosis, to help with the development of Turner syndrome in girls, and for other indications. It is taken orally.

Oxandrolone side effects include masculinisation symptoms such as acne, increased hair growth, voice changes and increased libido. The drug is a synthetic androgen and anabolic steroid, hence it is an androgen receptor (AR) agonist, a biological target of androgens such as testosterone and dihydrotestosterone. It has strong anabolic effects and weak androgenic effects, giving it a mild side effect profile and making it particularly suitable for use in women.

Oxandrolone was first described in 1962 and was introduced for medical use in 1964. It is used primarily in the United States. In addition to its medical use, oxandrolone is used to improve physique and performance.

Medical use

Oxandrolone has been researched and prescribed to treat a wide variety of conditions. It is FDA approved for the treatment of bone pain associated with osteoporosis, for weight gain after surgery or physical trauma, during chronic infection or in the context of unexplained weight loss, and to counteract the catabolic effects of long-term corticosteroid therapy. As of 2016, it is often prescribed off-label to speed recovery from severe burns, aid the development of girls with Turner syndrome, and counteract debilitation caused by HIV/AIDS. Oxandrolone improves both short and long term outcomes in people recovering from severe burns and is well established as a safe treatment for this indication. It is also used in the treatment of idiopathic stunting, anaemia, hereditary angioedema, alcoholic hepatitis and hypogonadism.

Medical studies have established the effectiveness of oxandrolone in the development of girls with Turner syndrome. Although oxandrolone has long been used to accelerate the growth of children with idiopathic stunting, it is unlikely to increase adult height, and in some cases may even decrease it. Therefore, oxandrolone has largely been replaced by growth hormone for this use. Children with idiopathic stunting or Turner syndrome are prescribed doses of oxandrolone much lower than those prescribed to people with burns to minimise the likelihood of virilisation and prematurity.
